Job Summary

We are seeking a skilled and compassionate Home Health LPN to join our team at HealthONE Healthcare at Home. As a Home Health LPN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities
  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Work collaboratively with the healthcare team to ensure the provision of quality patient care.
  • Perform ongoing assessments during each visit and document data in patient medical records.
  • Communicate significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician.
  • Provide basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs.
Requirements
  • Vocational School Graduate
