Responsibilities:- Administer skilled nursing care to patients in their homes, under the direction of a Registered Nurse and/or Director, Professional Services.
- Perform all functions in a professional and ethical manner, collaborating with other members of the healthcare team to ensure quality patient care.
- Verify the Plan of Care prior to each visit and provide care according to physician orders, assessment data, established standards, and guidelines.
- Document all findings, communications, and interventions, as well as patient responses and nursing interventions.
- Provide basic and specialized teaching as ordered in the Plan of Care and based on patient needs.
- Vocational School Graduate
