Join Our Team as a Home Health RN

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at HealthONE Healthcare at Home. As a Home Health RN, you will play a vital role in providing high-quality patient care and support to individuals in the comfort of their own homes.

About the Role

As a Home Health RN, you will be responsible for assessing patients' physical, psychosocial, and environmental needs, developing individualized plans of care, and collaborating with the interdisciplinary team to achieve patient goals. You will also supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.

Key Responsibilities
  • Assess patients' needs and develop individualized plans of care
  • Collaborate with the interdisciplinary team to achieve patient goals
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs
  • Perform OASIS, assessment, and visit paperwork according to agency policy
  • Communicate significant findings, problems, and changes to the Clinical Manager and physician

Requirements
