Responsibilities- Provide basic bedside care to patients, including vital sign monitoring and medication administration.
- Assist patients with daily living activities, such as bathing, dressing, and personal hygiene.
- Communicate effectively with patients and families to understand their needs and concerns.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Graduate of an approved school of nursing and licensed as a Licensed Practical Nurse in the state of employment.
- Valid state driver's license, automobile liability insurance, and dependable transportation.
- Minimum of one year of clinical experience, with previous home health or hospice experience preferred.
