Position Overview

The role involves delivering expert social work services to patients and their families within designated areas of the hospital and clinics, as well as providing support in other areas as needed. The aim is to enhance the psychosocial well-being of patients and families while working under provisional supervision. Building and maintaining strong, credible relationships with patients, medical staff, and nursing personnel is essential to ensure a collaborative approach and effective continuity of care.

Key Responsibilities

  • Conducting Psychosocial Assessments:
    • Perform daily screenings for patients with new chronic illness diagnoses, complex medical needs, or safety concerns.
    • Establish professional relationships with families and staff.
    • Collaborate with multidisciplinary teams to assess the medical needs of patients and families.
    • Document assessments accurately in patient charts.
    • Develop patient goals and monitor care plans using strength-based interventions.
    • Provide relevant patient education and record it in the patient's electronic medical record (EMR).
    • Communicate assessments and care plans with the patient care team.
    • Evaluate high-risk families and provide interventions focused on safety and mental health, collaborating with the healthcare team to ensure a secure environment.
  • Referrals and Resource Coordination:
    • Collect information from patients and families.
    • Share appropriate resources and information with patients and families.
    • Maintain current knowledge of community resources related to patients' medical conditions and social situations.
    • Utilize population health management tools to enhance patient and family well-being.
    • Assist in addressing social determinants of health.
    • Advocate for access to mental health, financial resources, and other community services.
    • Ensure follow-up and continuity of care, documenting interventions in patient charts.
  • Crisis Intervention and Support:
    • Provide crisis counseling and support to patients and families, assisting with problem-solving and adjustment to illness.
    • Offer interventions that reflect an understanding of how illness affects social well-being.
    • Document interventions appropriately and provide recommendations to healthcare team members.
    • Facilitate support groups as necessary.
  • Child Welfare Assessments:
    • Assess psychosocial dynamics in cases of suspected child abuse or neglect.
    • Communicate assessments and plans with medical and nursing staff.
    • Facilitate referrals and maintain communication with Child Protective Services (CPS) and law enforcement.
    • Document relevant information and complete reports promptly.
  • On-Call Services:
    • Provide on-call support for the Social Work Department as a provisionally licensed Social Worker.
    • Assist with trauma cases in the Emergency Department, responding to calls promptly and professionally.
  • Consultation on Legal Matters:
    • Offer guidance on legal issues such as guardianship and adoption cases.
    • Consult with legal counsel as necessary regarding state wards and consent for treatment.
  • Transplant Program Participation:
    • Depending on departmental needs, participate in transplant program functions, including psychosocial assessments of patients and families.

Qualifications

  • Education: Master's Degree in Social Work from an accredited institution is required.
  • Experience: 1-3 years of supervised clinical social work experience is preferred, along with experience in medical social work.
  • Skills: Knowledge of human behavior, social issues, counseling techniques, and community resources is essential. Strong verbal and written communication skills are required to relay case information effectively.
  • Licenses: Current Provisional Master Social Worker and Provisional Mental Health Practitioner licenses in Nebraska are required, along with a valid driver's license and a clean driving record.
