  • Facilitated pharmaceutical care services, ensuring drug utilization activities align with patient care needs, evidence-based best practices, and regulatory standards.
  • Completed critical patient monitoring and reviewed patient profiles to identify, prevent, or mitigate drug-related problems.
  • Made appropriate, evidence-based medication recommendations, participating in the management of medical emergencies and discharge medication review.

**Required Skills and Qualifications**

  • Bachelor's degree in Pharmacy or Doctor of Pharmacy required.
  • ASHP accredited PGY2 infectious disease residency or equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship required.
