This position focuses on providing essential services to youth who have been reunited with their families after an out-of-home placement. As a Community Support Specialist, you will collaborate closely with designated families to tackle a variety of issues, including safety planning, parenting techniques, and strategies for managing challenging behaviors. Additionally, you may assist families in areas such as household management, nutrition, stable housing, education, employment, medical care, mental health support, and substance abuse resources.
We welcome both seasoned professionals and recent graduates eager to embark on a career in the human and social services sector. If you are prepared to embrace a rewarding challenge and positively influence the lives of others, we encourage you to consider this opportunity.
Key Responsibilities:
- Assess youth and families to identify treatment needs and connect them with appropriate resources.
- Review client histories and capabilities to determine necessary services for managing safety concerns.
- Conduct face-to-face meetings with families to ensure adherence to safety plans and provide crisis intervention to support and educate.
- Evaluate family situations and refer them to relevant community resources.
- Utilize sound judgment to advocate for clients during emergencies and crises.
- Provide safe transportation for clients when necessary.
Work Environment:
This position involves local travel to meet clients in their homes or other community settings. Flexibility for hybrid or remote work is available for two days each week.
Work Schedule:
The schedule includes Monday through Friday with rotating evening and weekend shifts. One evening shift per week and one weekend shift per month are required.
