Pediatric LPN Private Duty Evening Nurse

We are seeking a skilled and compassionate Pediatric Licensed Practical Nurse to join our team in providing high-quality care to patients in their homes. As a Private Duty Pediatric LPN, you will be responsible for delivering direct nursing care to acutely ill and chronically ill patients, while also coordinating other patient care activities and services to ensure the highest level of patient care.
Key Responsibilities- Provide high-quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
- Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
- Ensure confidentiality of patient health information per agency policy.
- Current LPN licensure in the state of practice.
- Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
