Responsibilities:
  • Facilitate residents' linkage with community services, including case management, clinical services, and day programs.

  • Provide individual counseling and support services, including housing assistance and daily living skills training.
  • Document all client interactions and progress.
  • Attend weekly supervision and staff meetings.
  • Remain current with all Agency required compliance documents and trainings.
Requirements:
  • Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health discipline.
  • 2 years of experience working in a similar setting and/or with individuals with mental illnesses.
  • Excellent verbal and written communication skills.
