Job Summary

The Home Health Field RN is a nursing professional care provider under the direction of the director or designee who utilizes the nursing process, nursing standards and physicians orders to plan, provide and direct nursing practice. This role requires the ability to accomplish nursing practice that includes practice elements as defined by the Kentucky Board of Nursing and in accordance with the Ephraim McDowell Health nursing professional practice model. The successful candidate will be responsible for providing patient and family centered care as central to practice and a foundation of caring, compassion, competence, communication, collaboration and teamwork. Additionally, this role requires leadership skills to participate in shared nursing leadership to accomplish positive outcomes, education, respect, dignity and safety for the patient and their family. The Home Health Field RN is expected to exhibit F.I.R.S.T. values (Friendliness, Innovation, Respect, Service and Trust).

This role involves performing hands-on nursing care as well as planning, organizing, and directing home care services. The Home Health Field RN is experienced in nursing with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.

Working Conditions, Hazards and Physical Effort

This role requires working in a well-lighted, ventilated and air conditioned area when in the office. The schedule may consist of weekends, holidays and shift work. The successful candidate will require excellent observation, communication, and problem solving skills as well as basic math skill and nursing skills. Additionally, this role requires physical endurance for prolonged periods in a stationary position and moving around the unit. The candidate will be at risk of exposure to disease, infection and physical strain and assault. The role also involves risks exposure to blood, body fluids, and tissue. The candidate must be able to move, position, and/or transfer patients as well as supplies and equipment. The candidate must be able to move or position 50 lbs. independently.


The candidate must be a licensed driver with an automobile that is insured in accordance with state and Organization requirements and is in good working order.

Minimum Qualifications or Equivalents

Education:
Graduate of an accredited ADN, BSN, MSN, or doctorate nursing program. BSN preferred.

Experience:

One (1) year home health experience and/or one to two (1-2) years of recent acute care experience in an institutional setting preferred.
