A Nurse Practitioner plays a vital role in delivering advanced nursing care to patients and their families, while also serving as a clinical resource for the healthcare team.
Key Responsibilities
- Delivers independent primary care, offering consultation and expert advice as a specialized practitioner.
- Conducts patient visits, manages on-call responsibilities, and exercises prescriptive authority in collaboration with the supervising physician, leading interdisciplinary team meetings.
- Coordinates care for patients with complex medical conditions, as requested by the healthcare team.
- Performs thorough assessments for patients with intricate health issues and develops comprehensive care plans.
- Facilitates face-to-face patient encounters as needed, in accordance with collaborative agreements with physicians.
- Maintains accurate documentation and records of all services provided, adhering to established company processes and forms.
- Provides clinical education internally and externally, as required.
- Actively participates in team and staff meetings, contributing to collaborative discussions.
- Acts as a clinical resource for all team members, enhancing overall patient care.
- Engages in quality improvement initiatives and performance enhancement activities as requested.
- Undertakes additional responsibilities as assigned.
License Requirements
- Current CPR certification is mandatory.
- Must hold a valid certification as a clinical Nurse Specialist or Nurse Practitioner in the practicing state.
- Certification by ANCC or AANP is required.
- Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
- NPI Number is necessary.
- DEA registration with prescribing privileges is required.
- Must be enrolled in PECOS.
- CAQH enrollment is required.
- Medicare and Medicaid numbers are needed (Medicaid is preferable but not mandatory).
- Minimum of one year of Nurse Practitioner experience in chronic disease management, family, or primary care is preferred.
- For states including AL, AR, AZ, CA, CO, CT, D.C., DE, FL, GA, ID, IL, IN, KY, LA, MA, MD, MI, MN, MO, MS, NC, NH, NJ, NM, NY, NV, OH, OK, OR, PA, RI, SC, TN, TX, VA, WA, WI, WV: No additional state-specific requirements apply.
