Home and Supportive Care Nurse Practitioner or Physician Assistant Opportunity

OptumCare is seeking a skilled Home and Supportive Care Nurse Practitioner or Physician Assistant to join our team in San Antonio, TX. As a member of our team, you will have the opportunity to work with a multi-disciplinary team of healthcare providers, including Primary Care Physicians, RN Case Managers, social workers, and clinic staff.

Key Responsibilities:
  • Provide comprehensive care to patients in their homes, including managing acute and chronic conditions, such as diabetes, hypertension, and kidney disease.
  • Diagnose and treat patients with minor injuries and other acute medical conditions.
  • Facilitate patient and family decision-making regarding treatment decisions, end-of-life care, and serious illness discussions.
  • Prescribe pharmacological and non-pharmacological interventions as needed.

Requirements:
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Current licensure through the Texas Board of Nurse Examiners or Licensed Physician Assistant by the Texas State Medical Board.
  • Valid DEA license or the ability to obtain one prior to employment.
  • 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant.
  • Comfortable managing a primarily Medicare-aged patient population.
  • Reliable transportation.
