Home and Supportive Care Nurse Practitioner or Physician Assistant

Optum Medical Group is seeking a skilled Home and Supportive Care Nurse Practitioner or Physician Assistant to join our team in San Antonio, TX. As a member of our team, you will have the opportunity to work with a multi-disciplinary team of healthcare providers to deliver high-quality care to our patients.
Key Responsibilities:- Provide primary care to patients in their homes, focusing on managing chronic conditions and preventing hospitalizations
- Collaborate with other healthcare professionals to develop and implement individualized care plans
- Diagnose and treat acute and chronic conditions, including diabetes, hypertension, and kidney disease
- Prescribe medications and other interventions as needed
- Facilitate patient and family decision-making regarding treatment options and end-of-life care

- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current licensure through the Texas Board of Nurse Examiners or the Texas State Medical Board
- Valid DEA license or ability to obtain one prior to employment
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
- Comfortable managing a primarily Medicare-aged patient population
