Home and Supportive Care Nurse Practitioner or Physician Assistant

At Optum, we're seeking a skilled Home and Supportive Care Nurse Practitioner or Physician Assistant to join our team in San Antonio, TX. As a member of our OptumCare organization, you'll be part of a clinician-led care team that's changing the way healthcare works.
Key Responsibilities:- Provide high-quality, patient-centered care to Medicare-aged patients in their homes
- Manage complex medical conditions, including diabetes, hypertension, and kidney disease
- Collaborate with a multidisciplinary team to develop and implement care plans
- Prescribe medications and non-pharmacological interventions as needed
- Facilitate patient and family decision-making regarding treatment options and end-of-life care

- Certification through AANP, ANCC, or NCCPA
- Current licensure through the Texas Board of Nurse Examiners or Texas State Medical Board
- Valid DEA license or ability to obtain one prior to employment
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
- Comfortable managing a primarily Medicare-aged patient population
- 3+ years of clinical experience as a Nurse Practitioner or Physician Assistant
- Home health experience
- Bilingual (English/Spanish) fluency
- A working understanding of managed care environments
