Home and Supportive Care Nurse Practitioner or Physician Assistant

At Optum, we're seeking a skilled Home and Supportive Care Nurse Practitioner or Physician Assistant to join our team in San Antonio, TX. As a member of our OptumCare organization, you'll be part of a clinician-led care team that's changing the way healthcare works.

Key Responsibilities:
  • Provide high-quality, patient-centered care to Medicare-aged patients in their homes
  • Manage complex medical conditions, including diabetes, hypertension, and kidney disease
  • Collaborate with a multidisciplinary team to develop and implement care plans
  • Prescribe medications and non-pharmacological interventions as needed
  • Facilitate patient and family decision-making regarding treatment options and end-of-life care

Requirements:
  • Certification through AANP, ANCC, or NCCPA
  • Current licensure through the Texas Board of Nurse Examiners or Texas State Medical Board
  • Valid DEA license or ability to obtain one prior to employment
  • 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
  • Comfortable managing a primarily Medicare-aged patient population
Preferred Qualifications:
  • 3+ years of clinical experience as a Nurse Practitioner or Physician Assistant
  • Home health experience
  • Bilingual (English/Spanish) fluency
  • A working understanding of managed care environments
