About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.

Key Responsibilities:

  • Administer skilled nursing care to clients requiring professional services in their homes
  • Teach and supervise the family and other members of the nursing team
  • Act as primary liaison between physician, patient/family, and home health team
  • Provide direct patient care services to patients and supportive care to the patient's family
  • Manage and collaborate with an interdisciplinary group to develop and implement patient care plans

Requirements:

  • Current RN license in the state of employment
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing
  • Current driver's license and automobile insurance
  • Current CPR certification
