Overview:
The Home Study and Post-Release Services (HSPRS) Case Manager Level 2 is responsible for delivering comprehensive case management and support services to unaccompanied immigrant minors who have been released from federal custody. This role emphasizes a client-centered and trauma-informed methodology in collaboration with program staff and supervisors.
Key Responsibilities:
- Conduct home assessments and maintain communication with caregivers to evaluate the safety and suitability of placements, while linking children to essential community services.
- Provide ongoing case management focused on identifying and addressing the child's needs across various domains, including education, health, legal matters, and independent living.
- Perform thorough assessments in a child-centered, trauma-informed manner, ensuring accurate documentation in the case management system.
- Offer crisis intervention services as necessary, or connect families with appropriate resources to safeguard the well-being of minors during emergencies.
- Establish and nurture relationships with key governmental and non-profit organizations to enhance service delivery.
- Adhere to all program policies and procedures, ensuring operational efficiency.
- Meet or exceed productivity expectations consistently.
- Other duties as assigned.
- Exemplify exceptional customer service by prioritizing the needs of children, families, and clients in alignment with our mission to empower individuals and communities.
Qualifications:
- Minimum of 1 year of relevant experience, preferably with vulnerable or migrant populations; 3 years is preferred.
- Fluency in both Spanish and English, with strong written and verbal communication skills.
- Ability to manage confidential information responsibly.
- Willingness to travel domestically up to 60% of the time.
- Proficiency in Microsoft Office applications.
Education: Bachelor's degree in education, psychology, sociology, or related behavioral sciences.
Attendance: Regular and acceptable attendance is required.
Licenses: Valid driver's license with a clean record.
Vehicle: Access to a vehicle for daily use is necessary.
Other: Availability and willingness to travel as needed for business purposes.
