Job Summary

We are seeking a skilled Case Management Coordinator to join our team at CVS Health. In this role, you will play a critical part in delivering enhanced human-centric healthcare to our members.

As a Case Management Coordinator, you will utilize your critical thinking and judgment to collaborate and inform the case management process. You will facilitate appropriate healthcare outcomes for members by providing assistance with appointment scheduling, identifying and assisting with accessing benefits and education for members through the use of care management tools and resources.

Key Responsibilities
  • Conduct comprehensive evaluations of referred members' needs/eligibility and recommend approaches to case resolution and/or meeting needs.
  • Identify high-risk factors and service needs that may impact member outcomes and care planning components with appropriate referral to clinical case management or crisis intervention as needed.
  • Coordinate and implement assigned care plan activities and monitor care plan progress.
  • Enhance medical appropriateness and quality of care by consulting with case managers, supervisors, Medical Directors, and/or other health programs to overcome barriers to meeting goals and objectives.
  • Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices.
  • Help members actively and knowledgeably participate with their providers in healthcare decision-making.
  • Monitor, evaluate, and document care in compliance with regulatory and accreditation guidelines and company policies and procedures.
Requirements

We are looking for a candidate with 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook) and case management and discharge planning experience. A Bachelor's degree or non-licensed master level clinician with either degree being in behavioral health or human services is preferred (psychology, social work, marriage and family therapy, counseling) or equivalent experience.

This is a full-time telework position open to candidates in the United States. Schedule for this position is Monday-Friday 8:00a-5:00p in the assigned market time zone.
