Role Summary: We are looking for a dedicated and skilled Licensed Practical Nurse (LPN) who is fluent in Spanish to enhance our team. The ideal candidate will possess a minimum of one year of experience in home health care and demonstrate proficiency in caring for patients with tracheostomies and gastrostomies (g-tubes).

Key Responsibilities:

  • Deliver exceptional nursing care to clients at home, following the prescribed care plan.
  • Monitor and document patient vital signs, administer medications, and perform necessary nursing tasks.
  • Assist with tracheostomy management, including suctioning and observation.
  • Provide care for gastrostomy (g-tube) patients, including feeding and medication administration.
  • Effectively communicate with clients, families, and the healthcare team to ensure optimal care and outcomes.
  • Maintain precise and comprehensive patient records.
  • Educate clients and their families on health maintenance and disease prevention strategies.
  • Comply with all safety and infection control protocols.

Qualifications:

  • Current New Jersey LPN license.
  • At least one year of experience in home health care.
  • Experience with tracheostomy and gastrostomy (g-tube) care.
  • Fluency in Spanish is essential.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively.
