Position Overview
The Hospice Licensed Practical Nurse (LPN) is an essential member of the healthcare team, providing compassionate care to patients in need of hospice services. This role reports directly to the Director of Nurses and plays a critical part in enhancing the quality of life for terminally ill patients.

Key Responsibilities
The Hospice LPN is responsible for:
1. Delivering direct patient care as outlined in the State Nurse Practice Act.
2. Implementing nursing practices based on patient assessments and approved care plans.
3. Maintaining accurate and timely documentation of patient services.
4. Educating patients and their families regarding diagnoses and care plans.
5. Collaborating with interdisciplinary team members to coordinate hospice services.
6. Utilizing medical equipment and supplies efficiently.
7. Managing pain and symptoms effectively, while evaluating patient responses to treatments.
8. Engaging in personal and professional development, as well as participating in quality management initiatives.
9. Performing additional duties as assigned by the supervising registered nurse.

Qualifications
To be considered for this role, candidates must:
- Have graduated from an accredited practical nurse or vocational nursing program.
- Hold a current license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) through the state Board of Nursing.
- Preferably possess at least one year of clinical experience in hospice, home health, medical, surgical, or critical care settings.
- Understand the philosophy of hospice care and the needs of terminally ill patients.
- Be at least 18 years old and maintain good physical and mental health.
- Pass pre-employment background and health screenings.
- Have a valid driver's license and a reliable, insured vehicle.
- Exhibit strong verbal and written communication skills, along with organizational abilities.
