Position Overview
The Hospice Licensed Practical Nurse (LPN) is essential in delivering compassionate care to patients in a hospice setting.

Reporting Structure
This role reports directly to the Director of Nurses.

Key Responsibilities
The Hospice LPN is responsible for:
1. Delivering direct patient care under the supervision of a registered nurse.
2. Following the established plan of care and providing necessary treatments.
3. Collaborating with the healthcare team to achieve positive patient outcomes.

Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) through the state Board of Nursing.
- Preferred: One year of clinical experience in hospice, home health, medical, surgical, or critical care settings.
- Understanding of hospice philosophy and the needs of terminally ill patients.
- Must be at least 18 years old and maintain good physical and mental health.
- Ability to pass pre-employment background and health screenings.
- Valid driver's license with a reliable, insured vehicle.
- Compliance with accepted professional standards and practices.
- Strong verbal and written communication skills, along with organizational abilities.

Core Duties
- Provide direct patient care as defined by the State Nurse Practice Act.
- Implement nursing practices based on patient assessments and approved care plans.
- Ensure accurate and timely documentation of patient services.
- Educate patients and families regarding diagnoses and care plans.
- Coordinate hospice services and report needs to interdisciplinary team members.
- Utilize equipment and supplies efficiently.
- Manage pain and symptoms effectively, evaluating patient responses to treatments.
- Engage in personal and professional development, contributing to quality management initiatives.
- Perform additional duties as assigned by the supervising registered nurse.
