About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys, Inc., a leading provider of home health, hospice, and high-acuity care. As a Home Health Care RN, you will play a vital role in delivering patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Coordinate and oversee all aspects of patients' home care, ensuring seamless delivery of services.
  • Collaborate with patient care teams to develop and implement comprehensive care plans, tailored to each patient's unique needs.
  • Educate families on patient condition, needs, and care, promoting informed decision-making and empowerment.
  • Perform head-to-toe physical assessments, checking vital signs and managing medications to ensure optimal patient outcomes.
  • Administer and maintain catheters, give injections, IV therapy, or tube feedings, and complete documentation in a timely and accurate manner.
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), providing guidance and support to ensure high-quality care.
  • Regularly communicate patient progress to the clinical manager, physician, and care team, ensuring a collaborative approach to patient care.
Requirements
  • One year of clinical experience as a Registered Nurse (RN), with a strong background in home health care.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Minimum six months of home health experience, with a proven track record of delivering high-quality patient care.
  • Current CPR certification and a valid driver's license, with reliable transportation and liability insurance.
  • Able to assess patient status and identify requirements relative to age-specific needs, with a strong understanding of general use and functions of the computer.
