Position Overview
The Hospice Licensed Practical Nurse (LPN) is an essential member of the healthcare team, responsible for delivering compassionate care to patients in a hospice setting.

Reporting Structure
This role reports directly to the Director of Nurses.

Key Responsibilities
The Hospice LPN is tasked with:
1. Delivering direct patient care in accordance with the State Nurse Practice Act.
2. Implementing nursing practices based on patient assessments and approved care plans.
3. Documenting patient services accurately and promptly to reflect care plans.
4. Educating patients and their families about diagnoses and self-care plans.
5. Coordinating hospice services and communicating patient needs to interdisciplinary team members.
6. Utilizing medical equipment and supplies efficiently.
7. Managing pain and symptoms effectively while evaluating patient responses to treatments.
8. Engaging in personal and professional development and participating in quality management initiatives.
9. Performing additional duties as assigned by the supervising registered nurse.

Qualifications
To be considered for this role, candidates must:
- Be a graduate of an accredited practical or vocational nursing program.
- Hold a current license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) through the state Board of Nursing.
- Preferably have at least one year of clinical experience in hospice, home health, medical, surgical, or critical care settings.
- Understand the philosophy of hospice care and the needs of terminally ill patients.
- Be at least 18 years old and maintain good physical and mental health.
- Pass pre-employment background and health screenings.
- Possess a valid driver's license and have access to a reliable, insured vehicle.
- Demonstrate strong verbal and written communication skills, along with effective organizational abilities.
