The Family Stabilization Service Model is designed for families, particularly those from Latino, Cape Verdean, and Portuguese-speaking backgrounds, where children are at risk of being placed outside the home due to safety issues stemming from neglect or abuse. The primary intervention consists of three weekly home visits or community-based wraparound services. This program is delivered by a multidisciplinary team that includes a Master's level clinician and a Clinical Parent Aide.

Key Responsibilities
  • Deliver high-quality services to families involved with DCF, adhering to the Comprehensive and Family Stabilization models.
  • Collaborate with parents and caregivers to meet DCF-established goals.
  • Conduct family-centered assessments and modify treatment plans as needed.
  • Develop youth and family-centered treatment plans in partnership with families and other service providers.
  • Address child safety and welfare issues while connecting families to community resources, advocating in schools, and addressing legal and health needs.
  • Identify community resources and foster natural supports for families to achieve treatment goals.
  • Provide additional visits, wraparound services, and 24-hour emergency support as required.
  • Maintain flexibility in scheduling visits, including evenings and weekends when necessary.
  • Engage in ongoing collaboration with clinicians, DCF social workers, schools, law enforcement, and other community providers.
  • Participate in DCF meetings to address ongoing concerns and monitor progress.
  • Document all interactions with families, including progress notes and collateral contacts.
  • Offer guidance on parenting skills and address unique stressors faced by Latino, Cape Verdean, and Portuguese families.
  • Assist families with transportation to appointments and services as needed.
  • Collaborate on risk management and safety plans with families.
  • Provide or coordinate crisis and emergency services for families.
  • Work with various stakeholders involved with the youth and family.
  • Report all emergencies or critical incidents to the Agency.
Administrative Duties
  • Stay updated on all Agency, Government, and Funding training and health requirements.
  • Complete necessary paperwork in a strength-based manner as per Agency guidelines.
  • Accurately submit timesheets and mileage forms in a timely manner.
Required Knowledge, Skills, and Abilities
  • Ability to effectively engage with the target population.
  • Strong organizational skills and ability to work independently.
  • Excellent verbal and written communication skills.
  • Sound judgment and decision-making abilities.
  • Goal-oriented service delivery.
  • Ability to work collaboratively as part of a team.
  • Bilingual proficiency in Spanish/English; Portuguese and Cape Verdean languages are a plus.
Minimum Qualifications
  • A Master's degree in a human services field (e.g., counseling, family therapy, social work, psychology) from an accredited institution.
  • Training in family therapy and experience working with youth and families.
  • Experience navigating child and family-serving systems and advocating for families in behavioral health.
  • Proven ability to be innovative and analytical in problem-solving.
  • Strong communication, administrative, and organizational skills.
  • Ability to work effectively with diverse populations and community agencies.
  • Willingness to support families 24/7 throughout the year.
  • Valid driver's license and access to a vehicle for program needs.
  • Bilingual Spanish/English is required; Portuguese and Cape Verdean are advantageous.
