Overview
As a vital member of the Integrated Care Team, the Clinician will deliver clinical recovery services to individuals enrolled in the DMH funded Adult Community Clinical Services program. This position, which may be classified as a Licensed Practitioner of the Healing Arts when applicable, involves leading clinical teams to create thorough assessments and treatment strategies that empower individuals to develop skills necessary for transitioning through various levels of service towards achieving self-sufficiency.
Key Responsibilities
- Deliver short-term individual and group therapy sessions.
- Participate in all transitional and clinical treatment meetings within the community, including enrollment, intake, discharge, and transfer meetings. Formulate and oversee transition plans to assist individuals facing life changes.
- Provide clinical guidance to individuals on the Integrated and GLE teams.
- Ensure compliance with program documentation standards (ACA, Treatment Plans, Quarterly/Annual Reviews, Assessments, referrals), completing all required paperwork for the ACCS and BH CP programs.
- Create Person-Centered Treatment Plans that set realistic and achievable goals, focusing on individual strengths, personal preferences, and cultural and linguistic needs, while involving the individual’s LAR and support network (Family, Friends, Clergy, etc.).
- Engage with referred individuals within 72 hours of referral to assess immediate needs and evaluate current risks.
- Develop Crisis and Risk Management Plans.
- Formulate Rapid Housing plans following a Housing First Model.
- Coordinate the completion of necessary evaluation tools (Self Sufficiency Matrix, Milestones of Recovery-MOR's, SBRIT, risk assessments, etc.).
- Design and conduct clinical training sessions for Integrated Teams and GLE staff.
- Administer medications to individuals according to MAP protocol, which may include providing medication coverage during staff emergencies. Conduct medication audits as assigned.
Additional Duties
- Engage in regularly scheduled supervision.
- Attend mandatory meetings and agency training sessions.
- Perform other responsibilities as assigned by the supervisor.
- Uphold and promote the principles and policies of the agency.
- Foster a sense of hopefulness and respect for individual rights.
- Collaborate effectively with all staff members.
- Represent the agency professionally in all community interactions.
Qualifications
- Master's Degree with Massachusetts LCSW, LICSW, LMHC, O.T. or RN preferred.
- Familiarity with the Recovery Model.
- Understanding of community resources.
- Proficient in computer skills, including web-based email and data entry into spreadsheets and word processing documents.
Skills
- Exceptional oral, written, and presentation abilities.
- Capability to work effectively within a multidisciplinary team.

Working Conditions
