Job Title: Assessment Coordinator

Join The Salvation Army USA Western Territory team as an Assessment Coordinator for the Older Alaskans Program. This role is responsible for introducing services to Medicaid Waiver clients, coordinating with care coordination systems, and promoting the Older Alaskans Program in the community and to individual clients.

Key Responsibilities:
  1. Provide information on OAP services and handle initial telephone intake for potential clients.
  2. Complete assessments to identify client needs and determine eligibility for services.
  3. Coordinate with care coordinators, family members, and other caregivers to determine client needs.
  4. Provide client follow-up as needed and arrange for home visits.
  5. Maintain accurate client case notes and files, and monitor waiting lists.
  6. Assist in planning and carrying out targeted outreach efforts.
  7. Promptly report any incidents of suspected or known elder abuse.
Requirements:
  • Associates Degree in Human Services or equivalent related work experience.
  • Ability to accurately report client functional, physical, social, and mental status.
  • Demonstrated skill and sensitivity in communicating with seniors, the disabled, and those who use English as a second language.
  • Proficient in using a computer and applications including Excel and Word.
  • Able to use basic office equipment.
  • Ability to maintain excellent working relationships and work collaboratively and effectively with others.
  • Current, valid Alaska driver's license.
Work Environment:

Work is performed in a well-lit, heated, and ventilated office environment. Noise level is generally low. The use of personal protective equipment may be necessary during periods of exposure to clients who are ill during home visits.

Language Skills:

Must be able to understand, follow and give both written and oral instructions. The employee must be able to use speech and hearing for ordinary and telephone conversation.

Mathematical Skills:

Must be able to perform simple calculations and apply concepts such as ratios and percentages in practical situations.

Performance Skills:

Must be creative, self-motivated, organized, flexible, and possess leadership skills.
