Job Summary:The Care Management Support Coordinator will support administrative care management activities, including performing outreach, answering inbound calls, and scheduling services. The ideal candidate will serve as a point of contact to members, providers, and staff to resolve issues and document member records in accordance with current state and regulatory guidelines.
Key Responsibilities:- Support administrative care management activities, including outreach, inbound calls, and scheduling services.
- Serve as a point of contact to members, providers, and staff to resolve issues and document member records.
- Perform data entry into True Care and navigate multiple systems.
- Make outbound calls to existing members to discuss care/medical reasons for signing up for the program.
- Enroll members into the Care Management program.
- Meet production/sales-based targets, with a minimum of 60 calls per day in a fast-paced environment.
- 1-2 years' experience in a fast-paced call center environment.
- At least 2 years of prior Healthcare experience.
- Ability to adapt to change.
- SALES experience highly preferred.
- Bilingual in Spanish a plus.
