Job Description

As a Patient Care Coordinator, you will be an extension of the provider, helping to close the gaps in patient care. You will build a rapport with patients through monthly calls, supporting them through services such as Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Physiological Monitoring (RPM). You will empower patients to meet their goals, guide them in their healthcare journey, and advocate for their needs by utilizing active listening skills, investigating patient concerns, documenting appropriately, and communicating professionally with their patient's care circle.

Key Responsibilities:

  • Manage and support patients' healthcare needs via onsite and phone conversations
  • Ensure that daily production goals and quality metrics are met within standards set forth by IronRod Health
  • Complete individualized patient care plans and perform care management and care coordination services via our software platform
  • Maintain frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes
  • Document all communications with patients, their caregivers, and their providers accurately
  • Communicate to and from Care Clinicians or office designee regarding patient needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
  • Adhere to documentation protocols and best practices for daily workflow, escalation of client issues, comprehensive care plans, and internal communications
  • Exhibit excellent customer service skills demonstrated by positive feedback from customers and patients
  • Be able to speak to the benefit and value of Care Management services, to ensure patient enrollment and retention goals are met

Requirements:

  • A Medical Assistant diploma, Nationally Certified Pharmacy Technician license, or LPN with compact license with at least 2 years' work experience in your role (required)
  • HIPAA compliance Experience
  • Experience and proficiency working with Electronic Medical Records (EMR) software as a user (required)
  • Knowledge of healthcare terms, conditions, roles, and basic care principles
  • An ability to develop and build trusting relationships with patients and partner practices
  • A true passion for caring for patients and guiding them in their health journeys
  • Ability to stay flexible and quickly adapt to new processes, software changes, and priorities
  • Candidate must be able to pass a background check, have a positive attitude, adapt positively to change, have a strong understanding of team dynamics, and a willingness to learn new skills on a continuous basis
  • Must be an empathetic and active listener with the ability to thrive independently in a fast-paced environment
