Key Responsibilities- Conduct comprehensive occupational therapy evaluations and assessments to identify participants' needs and develop personalized treatment plans.
- Provide occupational therapy services, including exercises, activities, and education, to help participants achieve their goals and maintain their functional abilities.
- Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each participant.
- Communicate effectively with participants, families, and healthcare professionals to ensure seamless care coordination and optimal outcomes.
- Document all services provided, including progress notes, treatment plans, and discharge summaries, in accordance with InnovAge's documentation standards.
- Participate in peer review and quality improvement activities to ensure the highest level of care and service delivery.
- Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain expertise and provide evidence-based care.
- Master's degree in Occupational Therapy (MOT) or Doctorate in Occupational Therapy (OTD) from an accredited program.
- Current licensure as an Occupational Therapist in the state where you will be practicing.
- National certification as an Occupational Therapist (OTR) or eligible for certification.
- At least one year of experience working with the elderly population, preferably in a geriatric care setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to develop and implement effective treatment plans.
- Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
- A competitive salary range of $53.50-$64.00 per hour.
