What We Offer
  • We offer an estimated salary range of $65,000-$85,000 per year, depending on experience and location.
Job Responsibilities

Your primary responsibility as a Registered Nurse Home Health will be to provide skilled nursing care to patients in their homes. This includes assessing patients' physical, emotional, and social needs, developing individualized plans of care, and collaborating with healthcare teams to ensure seamless transitions of care.

  • You will work closely with patients, families, and caregivers to identify health goals and priorities.
  • You will assess patients' conditions, develop care plans, and implement interventions to improve their overall well-being.
  • You will maintain accurate records of patient interactions, assessments, and care provided.
  • You will communicate effectively with healthcare teams, including physicians, therapists, and other nurses, to coordinate patient care.
Requirements
  • You must have a current, unrestricted state RN license.
  • You must have at least 1 year of experience in a clinical care setting as a nurse.
  • You must be able and willing to travel within your branch/office coverage area.
  • You must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
