Nursing Professional for Geriatric Care

We are seeking a skilled Nursing Professional to join our Home Care Team at Hebrew Senior Life. As a key member of our clinical support team, you will play a vital role in delivering high-quality care to patients in their homes.

Responsibilities
- Ensure all care delivered meets established standards of professional nursing practice.
- Collaborate with other members of the clinical support team to meet the clinical needs of patients and monitor outcomes of care.
- Adhere to best practice recommendations and implement evidence-based interventions to ensure optimum outcomes for patients.
- Collaborate and participate in care planning activities.
- Evaluate and revise plans of care as necessary to ensure they accurately reflect individualized approaches to patient needs.
- Document all interactions and interventions in the electronic medical record.
- Identify and seek out necessary resources to ensure safe and effective care delivery.
- Delegate and supervise clinical care delivered by licensed practical nurses and nursing assistants.
- Acknowledge accountability for own learning needs and professional development.
- Demonstrate critical thinking abilities in problem resolution.
- Model professional behavior and promote a positive work environment.
- Participate in shared governance, committees, and performance improvement projects.
- Develop new work processes and systems in collaboration with others.
- Serve as a resource and support to patients and families.
- Maintain compliance with policies, procedures, and regulatory matters.
- Contribute to the collection and monitoring of quality data to ensure high standards of care.
Requirements
- Graduate of an approved school of nursing.
- Previous experience preferred.
- Must possess excellent communication skills, be proactive, collaborative, and results-oriented.
- Ability to adapt to changing priorities and display sound judgment.
- Positive organizational skills and ability to manage multiple projects simultaneously.
- Motivated to learn and flexible to change.
- Computer literacy required.
Estimated Salary: $65,000 - $85,000 per year, depending on experience and qualifications. We offer a competitive benefits package, including health insurance, retirement plan, and paid time off.
