Compassionate Care Specialist

BAYADA Home Health Care is seeking a skilled and dedicated Registered Nurse (RN) to provide exceptional care to adult and geriatric clients in the comfort of their own homes in Dedham, Massachusetts.
About the RoleAs a Compassionate Care Specialist, you will be responsible for delivering high-quality, personalized care to our clients, ensuring their safety, comfort, and independence. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans that address each client's unique needs and goals.
Your primary duties will include:
- Providing direct patient care, including wound care, medication administration, and treatments
- Conducting thorough assessments of clients' physical and emotional needs
- Developing and implementing individualized care plans in collaboration with clients, families, and healthcare teams
- Maintaining accurate records of client interactions, including progress notes and care plans
- Communicating effectively with clients, families, and healthcare professionals to ensure seamless care coordination
To be successful as a Compassionate Care Specialist, you must possess:
- A current license as a Registered Nurse in Massachusetts
- Graduation from an accredited and approved nursing program
- Prior clinical experience as a licensed RN, preferably in sub-acute or acute settings
