Job Summary

We are seeking a highly skilled Discharge Coordinator to join our clinical services team at Copper Springs. This role requires active involvement in the treatment planning process, providing aftercare coordination, and maintaining up-to-date community resources.

Key Responsibilities
  • Provide aftercare coordination for patients needing ongoing medical/psychosocial care following discharge.
  • Create and maintain up-to-date aftercare coordination community resources.
  • Display active involvement in treatment planning by setting up aftercare appointments as identified by the patient's provider and therapist.
  • Offer resource information to patients and families for future care needs.
  • Communicate proactively with clients, families, aftercare providers, and referral sources.
  • Develop detailed Discharge Summaries and Contracts for each patient.
  • Collaborate effectively with the interdisciplinary team, including physicians, utilization review, and nursing staff members.
  • Ensure accurate and timely completion of hospital and client documentation.
Requirements
  • Bachelor's degree in social work, counseling, psychology, or relevant state licensure (or equivalent experience).
  • Prior experience working with psychiatric and chemical dependency patients.
  • CPR certification and Crisis Prevention Training within 30 days (if required).
