Nurse Care Manager - First Connect
About the Role:We are seeking a highly skilled Nurse Care Manager to join our team at First Connect. As a Nurse Care Manager, you will play a crucial role in providing high-quality patient care and ensuring seamless coordination between healthcare providers.
Responsibilities:- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure timely and effective communication.
- Provide education and support to patients and their families on disease management and self-care strategies.
- Maintain accurate and up-to-date records of patient progress and outcomes.
- Current unencumbered Oregon RN license.
- Healthcare Provider Level AHA BLS certification.
- At least one year of nursing experience.
- Certified Addiction RN required.
- A competitive salary range of $70,000 - $90,000 per year.
