We are seeking a compassionate and experienced RN Case Manager to join our team at Serenity Hospice – an Addus family company. Based in Eugene and surrounding counties, this role offers a unique opportunity to make a meaningful difference in the lives of patients and their families.

Salary: $85,000 - $100,000 per year, depending on experience.

About the Role

This is a full-time position working Monday through Friday, 8am to 5pm. As an RN Case Manager, you will assume primary responsibility for patient and family caseloads, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.

You will be responsible for obtaining data and assessing necessary information from patient physical, psychological, social, and spiritual factors that may impact their needs and initiating intervention by other members of the interdisciplinary team.

Key responsibilities include:

  • Initiating communication with attending physicians, interdisciplinary teams, and other agencies to coordinate optimal care;
  • Providing regular visits to reassess patient needs and update plans of care;
  • Maintaining accurate patient records, charts, progress notes, reports, and preparing care plans timely;
  • Supervising and documenting delivery of care by Home Health Aides as required by Federal, State, and Company guidelines.
About You

To be successful in this role, you should have:

  • A graduate degree from an accredited registered nursing program;
  • A current RN license in the state of practice;
  • Current CPR and First Aid certification;
  • Knowledge of quality assessment and assurance procedures;
  • A valid driver's license and proof of insurance.
