Job Summary

Fortcom Health Services is seeking a Part-Time RN to join our team in Massachusetts. As a RN - Adult Foster Care Specialist, you will be responsible for providing high-quality care to our Adult Family/Foster Care program members.

Key Responsibilities
  • Conduct monthly home visits to assess the health status of members and develop individualized care plans.
  • Train and supervise AFC caregivers to ensure they are equipped to provide quality care.
  • Review and update member care plans as needed, in collaboration with the Program Director.
  • Monitor member health status and report any changes to their primary care physician.
  • Complete nursing progress notes and submit semiannual health-status reports to physicians.
  • Plan and implement discharges from the AFC program, as needed.
  • Provide ongoing training to AFC caregivers on health and aging topics.
Requirements
  • Associate or Bachelor's degree in Nursing.
  • Current RN licensure in Massachusetts.
  • At least two years of experience in home health care.
  • Bilingual skills (Spanish or Haitian Creole) are a plus.
