Licensed Vocational Nurse

Highland Park Rehabilitation & Nursing Center is seeking a skilled Licensed Vocational Nurse to join our team.
Job Summary:We are looking for a dedicated and compassionate Licensed Vocational Nurse to provide high-quality nursing care to our residents. As a Licensed Vocational Nurse, you will be responsible for supervising, providing, and coordinating nursing care in compliance with facility policies and procedures.
Responsibilities:- Supervise and evaluate direct resident care, initiating corrective action as necessary.
- Interpret policies and procedures to nursing assistants, residents, families, and physicians.
- Obtain report from nurse being relieved and record sufficient information for follow-up action.
- Provide report to nurse coming on duty, including sufficient information for follow-up action.
- Take appropriate action to ensure adequate staffing to meet residents' needs.
- Conduct resident rounds daily, reporting problems to Nursing Supervisor and initiating corrective action.
- Follow facility admission procedures and communicate resident admission to other departments, physicians, and ancillary service providers.
- Competently perform basic nursing skills.
- Follow facility discharge/transfer procedure and coordinate discharge/transfer with other departments, physicians, and ancillary service providers.
- Complete documentation related to residents' admission, discharge, or transfer.
- Supervise response to resident's call for assistance.
- Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Director of Nursing Service.
- Count narcotics, check emergency kit, and exchange keys.
- Check meal trays and nourishment to ensure proper diet is served and residents are assisted as necessary.
- Supervise the application and removal of all restraints and devices used to position and enable residents to function as independently as possible.
- Order, receive, and store medications appropriately.
- Notify physician, when necessary, of changes in resident's condition, weight, medication errors, medication not available, abnormal laboratory X-Ray results, and any other unusual observations.
- Document resident progress notes as required.
- Document daily notes on unstable residents, Medicare residents, and recently admitted residents.
- Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers.
- Obtain continuing education required to maintain licensure.
- Be responsible for the safety of residents under his/her supervision.
