Responsibilities

The RN Care Coordinator plays a critical role in overseeing the progression of care and discharge planning for patients requiring specialized services. This includes:

  • Collaborating with healthcare teams to ensure seamless transitions of care
  • Identifying and addressing patient needs, including cultural, language, and socioeconomic barriers
  • Developing and implementing effective care plans to promote quality outcomes and cost-effective care
  • Communicating with patients, families, and healthcare providers to ensure informed decision-making
Qualifications

To be successful in this role, candidates should possess:

  • Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing
  • California RN license
  • AHA BLS card
  • Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used
  • Critical thinking and problem-solving skills
  • Effective communication and collaboration skills
  • Knowledge of CMS standards and requirements
Preferred Qualifications

Candidates with the following qualifications are highly desirable:

  • Graduate of an accredited school of nursing (Bachelor's Degree in Nursing (BSN)) or related healthcare field
  • At least five years of nursing experience
  • Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification
  • Knowledge of managed care and payer environment
