Nurse Care Coordinator

Avenue Health is seeking a highly skilled and experienced nurse to support our Avenue Cares Health programs. As a key member of our team, you will play a vital role in delivering high-quality patient care and promoting health outcomes.
We are looking for a Bi-lingual (English/Spanish) speaking LPN/LVN with an active and unrestricted Arizona Compact nursing license to join our team. If you are passionate about making a difference in the lives of patients and have a strong background in medical-surgical acute care, post-op care, and discharge planning, we encourage you to apply.
As a Remote Patient Monitoring Specialist, you will be responsible for providing comprehensive patient assessments, developing whole-person-centered care plans, and monitoring physiological patient data alerts. You will also provide disease management education, share SDOH resources, and assist with care coordination needs.
Key Responsibilities:
- Provide top-of-license practice, delivering comprehensive patient assessments and developing whole-person-centered care plans.
- Monitor physiological patient data alerts, assess need for intervention, and escalate patient urgent care needs to physician.
- Direct patients as appropriate for emergency medical assistance needs.
- Collaborate with healthcare teams to ensure seamless care coordination and patient engagement.
Requirements:
- LPN/LVN with a current and unrestricted Arizona nursing or Compact Nursing license.
- Three-five years of Medical-Surgical Acute Care; Post-op care, discharge planning experience.
- Health Coach certification a plus.
- Remote-patient monitoring/telehealth experience a plus.
- Excellent communication and interpersonal skills.
Skills:
- Tech savvy with proficiency in: Internet Navigation, Smartphone technology, Microsoft Office Suite products (Word, Excel, and Outlook).
- Motivational Interviewing/Health Behavior Change experience a plus.
- Experience in designing whole-person, individualized patient care plans.
Other Requirements:
- Bi-lingual (English/Spanish)
- High Speed Internet access with at least 100 kbps upload and download bandwidth per line.
