Family Services Coordinator

We are seeking a highly skilled and compassionate Family Services Coordinator to join our team at Dignity Health - California Hospital Medical Center. As a key member of our interdisciplinary team, you will provide case management, teaching, and demonstration activities to support the well-being of children and families. Your expertise in child development, social work, and psychology will enable you to assess families across a broad spectrum of areas and provide referrals, advocacy, and support to promote stability and safety.
Responsibilities
- Provide case management services to a caseload of 10 to 11 families, including teaching and demonstrating activities according to the child and family's plan of care.
- Collaborate with a team of professionals to ensure child safety and stability, and provide parenting education activities to promote healthy family dynamics.
- Make home visits to low-income families, utilizing your knowledge of the agencies in the Pico-Union and downtown Los Angeles areas that serve this population.
- Develop and maintain accurate records, and write clear and concise notes and reports to articulate concepts and ideas.
- Utilize your bilingual English/Spanish speaking skills to communicate effectively with families and provide support in their preferred language.
Requirements
- Bachelor's degree in child development, social work, psychology, human development, or a related field.
- Wraparound training and experience working with low-income families and families from diverse cultural and ethnic backgrounds.
- Ability to function as a member of an interdisciplinary team, and assess families across a broad spectrum of areas recognizing professional boundaries and the need for consultation from other disciplines.
- Empathy, warmth, and the ability to establish ongoing supportive relationships with an ethnically diverse population.
- Possession of a valid California driver's license and ability to make home visits to various Los Angeles County locations.
