About the Role

We are seeking a highly skilled and compassionate Transplant Coordinator RN to join our team at Providence. As a key member of our Renal Transplant Program, you will play a vital role in ensuring the smooth transition of patients through the transplant process.

Key Responsibilities
  • Provide direct and indirect nursing care to transplant candidates, recipients, and living donors
  • Coordinate between patients, hospital staff, community physicians, and the transplant center
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the multidisciplinary transplant team to ensure seamless care coordination
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
Requirements
  • Graduate of a school of nursing or current enrollment in an accelerated program leading to a BSN or MSN degree
  • California Registered Nurse License upon hire
  • 1 year RN experience in an acute care setting or renal RN experience
Preferred Qualifications
  • Certification in Transplant Coordination (CCTC or CCTN) through the American Board of Transplant Coordinators
  • 1 year Organ transplant experience
