Key Responsibilities
The Case Worker will:
- Collaborate as part of the Community Services Division team.
- Engage in outreach efforts to identify candidates for Permanent Supportive Housing (PSH) and provide ongoing support to current residents.
- Coordinate essential services and meals for participants, including conducting intake assessments and crisis intervention in emergency situations.
- Assist the Case Manager in facilitating comprehensive case management activities.
The role includes the following essential functions:
- Ensure the safety and security of staff, participants, and volunteers through regular monitoring and compliance with safety protocols.
- Stay informed about emergency procedures related to building safety and client emergencies, ensuring a prompt response to crises.
- Conduct screenings, assessments, and crisis interventions, providing referrals to appropriate services such as hospitals or crisis lines.
- Effectively communicate organizational policies and regulations to participants, ensuring adherence to established procedures.
- Register participants and volunteers while maintaining accurate statistical records and documentation.
- Track statistical data diligently on a daily basis.
- Support volunteers by providing meals, hospitality, and assistance with program activities.
- Facilitate relocation assistance for clients as necessary.
- Manage incoming calls and document them in the designated system, offering referrals when appropriate.
- Accurately enter documentation into the management information system.
- Be prepared to work in various shelter programs as assigned.
- Maintain positive relationships with community professionals.
- Complete daily filing of discharged residential files as required.
- Assist with the discharge process for residential clients as directed.
- Adhere to all agency policies and procedures as well as those specific to the organization.
To be considered for this position, candidates should possess:
- A high school diploma or equivalent; a Bachelor's degree in a relevant field is preferred.
- A minimum of one year of experience in social work, crisis intervention, or counseling is advantageous.
- Must be at least 21 years of age.
- Proven ability to work effectively with individuals and families in crisis.
- Experience working with diverse populations.
- Strong interpersonal skills and the ability to engage with individuals in a compassionate, non-judgmental manner.
- Proficiency in Microsoft Office applications, including Outlook, Word, and Teams.
- Excellent verbal and written communication skills, with fluency in English.
- Successful completion of background checks and health screenings.
- A valid driver's license and reliable transportation.
