Qualifications
The candidate must possess a Master's Degree or its equivalent in social work, psychology, or counseling from an accredited institution. Previous supervisory experience is preferred. A valid NJ driver's license in good standing is required.

Applicants should exhibit the capability to foster a unified team, oversee and mentor the effective application of motivational interviewing and case management services, resolve conflicts, and collaborate with the ICMS team. Comfort in networking with various formal and informal community resources and supports is essential.

Key Responsibilities

  • Provide services tailored to meet the needs of consumers while facilitating the integration of physical and mental health. Ensure that case managers complete all necessary consumer electronic clinical record documentation accurately and promptly. Engage in local mental health and human services planning activities, identifying resource gaps in these areas.
  • Conduct assessments and contribute to the formulation and continuous evaluation of individual service plans.
  • Lead team meetings to assess service requirements and prioritize assistance for vulnerable individuals, establishing partnerships with community providers.
  • Maintain records as mandated by funding sources through electronic clinical record software and/or accreditation agencies.
  • Meet all expected productivity benchmarks concerning direct service delivery to consumers. Undertake additional responsibilities as assigned by the Program Director.
