Clinical Nutrition Specialist

FUNCTION:
The primary role involves delivering nutritional education, guidance, and interventions tailored to the needs of patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD). This position is crucial in promoting understanding and adherence to the facility's philosophy and policies.
KEY RESPONSIBILITIES:
1. Uphold the mission, objectives, and values of the facility.
2. Conduct and document comprehensive nutrition evaluations for all newly admitted ESRD patients, with follow-up assessments as necessary.
3. Collaborate with medical professionals to ascertain dietary requirements and nutrition plans for CKD and ESRD patients.
4. Regularly monitor patient progress, including monthly evaluations of blood chemistry, fluid balance, and nutritional status, implementing interventions as required.
5. Provide education to patients and their families regarding nutritional needs, meal planning, and available community resources.
6. Participate in the onboarding process for new dietitians.
7. Engage in interdisciplinary team meetings focused on patient care, quality improvement, and team dynamics.
8. Assist in the management of Nutrition Services during staff absences or increased patient census.
9. Ensure compliance with all State and Federal CMS regulations related to nutrition services for CKD and ESRD patients, supporting any CMS/Network studies or data collection efforts.
10. Contribute to educational initiatives at both the facility and patient levels.
11. Take a leadership role in the education and implementation of quality assessment and performance improvement (QAPI) processes.
12. Participate in the development and execution of the facility's operational strategy.
13. Commit to personal professional growth and maintain necessary credentials.
TOOLS AND EQUIPMENT:
Utilization of calculators, personal computers (Word/Excel/PowerPoint).

MINIMUM QUALIFICATIONS:
1. Registered Dietitian as recognized by the Commission on Dietetic Registration.
2. State licensure as a Dietitian (LD).
3. Minimum of one year of clinical nutrition experience as a registered dietitian.
4. Preferred experience with computer applications.
5. Preferred experience in dialysis.
