Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to serve as our MDS Coordinator. As a key member of our care team, you will be responsible for overseeing the Resident Assessment Instrument (RAI) process and conducting assessments and care plan coordination for our guests.

Responsibilities
  • Conduct thorough assessments of residents to identify their care needs and develop individualized care plans.
  • Collaborate with interdisciplinary teams, including physicians, nurses, social services, therapy, dietary, and activity staff, to ensure comprehensive care.
  • Review information from hospitals, consults, and outside agencies to inform assessment and care planning.
  • Maintain accurate and up-to-date records of resident assessments and care plans.
  • Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements and best practices in care coordination.
Requirements
  • Current LPN licensure in the state of employment.
  • Knowledge of the RAI process and Prospective Payment Process (PPS).
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
