Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Hospice team. As a LPN - Hospice Care Specialist, you will provide high-quality patient care and support to individuals and their families in a sensitive and respectful manner.

Key Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current on best practices and evidence-based care.

Requirements:

  • Current license to practice as a Licensed Practical Nurse (LPN) in the State of Ohio.
  • Graduate of a School of Practical Nursing.
  • Previous Hospice or Home Care experience preferred.
