Physiocare Hospice Nurse

Job Summary:
The Physiocare Hospice Registered Nurse is responsible for providing high-quality patient care to a geographically assigned caseload of patients from admission to discharge. This role requires the development and implementation of a home health plan that meets each patient's specific needs, while ensuring compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, and HHA policies and procedures.
Responsibilities:- Conduct admission assessments within 24-48 hours of referral or patient return to residence.
- Develop and implement a written plan of care for each patient.
- Collect accurate OASIS data at designated time periods.
- Assess patient condition, initiate plan of care, and re-evaluate as necessary.
- Utilize appropriate pain management techniques and educate patients and families on pain management.
- Perform patient care responsibilities considering patient age and standard of care.
- Demonstrate knowledge of medications and their administration based on patient age and clinical position.
- Follow medication rights and reduce the potential for medication errors.
- Provide skilled nursing care and obtain necessary physician orders.
- Collaborate with physicians as patient needs dictate.
- Support a culture of safety and quality.
- Initiate case conferences to discuss multidisciplinary team responsibilities and patient progress.
- Notify Intake Coordinator of patients requiring weekend or holiday visits and submit updated nursing care plans.
- Follow up on lab results and notify physicians of significant results.
- Establish patient schedules and indicate which patients may be seen by part-time RNs when caseload prohibits Case Manager from seeing all scheduled patients.
- Re-evaluate the need for continued care on an ongoing basis and initiate recertification as necessary.
- Complete patient documentation on time.
- Attend in-services and staff meetings as needed.
- Complete assigned training on time.
- Complete annual education requirements.
- Maintain patient confidentiality.
- Graduate of an accredited school of nursing.
- Current state licensure as a Registered Nurse.
- Experience as an RN commensurate with one of the following: acute care, home healthcare, community health, or outpatient facility experience within the last two years.
- Current CPR card.
- Valid state driver's license and reliable automobile.
- Proof of current automobile insurance.
