Hospice Admissions RN Opportunity

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Admissions team. As a Hospice Admissions RN, you will play a vital role in ensuring that patients and their families receive the highest quality care and support during this challenging time.

Key Responsibilities:
  • Execute the hospice admission process in compliance with federal and state regulations.
  • Advocate for patients and families to meet their stated needs and refer them to other resources as appropriate.
  • Develop trusting relationships with patients and families as an end-of-life care expert.
  • Maintain confidentiality and safety of patient/client clinical records.
  • Communicate effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding new admissions.
Requirements:
  • Must be a licensed registered nurse in the state of operation.
  • At least 2 years of experience in a hospice, home health, or other healthcare service delivery system setting.
  • Current CPR certification.
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.
