KEY RESPONSIBILITIES:
  • Welcome families into the program through orientation sessions and enrollment meetings, encouraging parental involvement in program governance through participation in committees and activities.
  • Conduct intake and needs assessments with families, addressing various areas such as housing, financial planning, employment, education, nutrition, legal matters, health, parenting, and daily living skills.
  • Uphold program structure and policies by orienting participants on rules and conducting case management visits.
  • Perform home visits with assigned families at least twice a year.
  • Interview clients to develop Individualized Family Partnership Agreements that meet program standards, focusing on strengths and strategies to overcome identified challenges.
  • Set, assess, and monitor client goals linked to measurable outcomes, collaborating with a multidisciplinary team to ensure service coordination.
  • Advocate for clients' needs.
  • Offer support, referrals, and guidance to parents for themselves and their children.
  • Collaborate with educational staff to promote school readiness for assigned children and families.
  • Organize and lead Parent Committee meetings.
  • Recruit families and children for participation in early learning programs, including those with diagnosed disabilities.
  • Monitor child attendance according to established procedures and assist families in maintaining consistent attendance.
  • Collect and maintain health and other necessary documentation to comply with program mandates.
  • Support families during transitions into and out of educational and childcare settings.
  • Maintain accurate case records, documenting all services provided, including case notes and referral logs. Enter required data into agency databases as necessary.
  • Provide emergency response management to address any crises or safety concerns that may arise.
QUALIFICATIONS:
  • 1-2 years of experience in human services or early childhood education.
  • Experience working with parents and young children is preferred.
  • Proficiency in Spanish, Portuguese, Khmer, or Arabic is a plus.
  • A passion for working with low-income, culturally diverse, and at-risk families.
  • Proof of MMR vaccination and recent physical examination.
  • Strong customer service, organizational, interpersonal, and communication skills.
  • Ability to manage multiple tasks in a fast-paced environment.
  • Proficient in basic computer software and data collection systems.
  • CORI/DCF review required prior to employment.
  • Exceptional crisis management abilities.
EDUCATION:
A Bachelor's Degree is required, preferably in a Human Services-related field.

LICENSES:
Valid Driver's License is necessary.
