Palliative Care Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our Palliative Care team in a 100% outpatient setting. Our team provides patient-centered care with a multidisciplinary approach, ensuring excellent outcomes for our patients.
Key Responsibilities:- Provide high-quality, patient-centered care in an outpatient setting
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
- Conduct face-to-face visits at care facilities and homes to ensure comprehensive care
- Enjoy a great work-life balance in a temperate climate with plenty of outdoor activities
- Be part of a provider-led group of hospital-employed providers
