About the Role

We are seeking a compassionate and skilled Nurse Practitioner to join our team in providing high-quality care to patients with serious illnesses. As a Hospice and Palliative Care Nurse Practitioner, you will work collaboratively with our interdisciplinary team to deliver comprehensive care that addresses the physical, emotional, and spiritual needs of our patients and their families.

Responsibilities
  • Provide primary and specialty care to patients with serious illnesses, including those with cancer, heart disease, and other life-limiting conditions.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with our team of physicians, nurses, social workers, and other healthcare professionals to ensure seamless care coordination.
  • Conduct thorough assessments, diagnose conditions, and develop treatment plans that align with the patient's values and goals.
  • Prescribe medications and order diagnostic tests as needed.
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
  • 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.
  • Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively.
